Advanced Care Planning
Nobody wants to think about getting ill and dying, the vast majority of us will use every excuse to avoid the conversation. This is especially true when having to think that your family members will no longer be here one day.

Unfortunately, we will all eventually die and we have no power to prevent or change this fact.
However, we do have the power to make sure that when we approach the end of our lives, we can have our wishes and preferences met to ensure we are as comfortable as possible and have a good death. The best way to ensure this is to discuss and plan for it in advance.
Advanced Care Planning (End of Life Care)
When we are well, we can:
1. Write your will
2. Record your funeral wishes
3. Plan your future care and support
4. Consider registering as an organ donor
5. Tell your loved ones your wishes

A majority of our residents cannot voice their wishes to us, so we rely on their loved ones to provide us with information so we can get it right. This is vital for us, as we want to get it right for both you and your loved ones.

We can do this by asking relatives and friends to complete an Advanced Care Plan that we will provide so we know what your loved one’s wishes are when the time comes for the next part of their journey.
